Community Health Workers (CHWs) encourage health and wellness by helping people in their communities engage in healthy behaviors. They support those who may have difficulty connecting to health resources and social services.
CHWs at the Howard County Health Department may be available to help:
- Support learning to community members with chronic disease and other health needs
- Encourage healthy lifestyle choices with families, pregnant and post-partum birthing people, and older adults, and/or
- Make referrals to community resources and social services
WORKGROUPS
Community Health Worker Learning Collaborative
The Community Health Worker Learning Collaborative (CHWLC) offers a peer learning opportunity for CHWs to share resources and facilitate discussion among partners who support and employ CHWs across the County. CHWs promote health and wellness by empowering individuals to adopt healthy behaviors and bridging gaps in access to culturally appropriate healthcare and social services

GROWING HEALTHY FAMILIES WORKGROUP
The Growing Healthy Families Workgroup's focus is to improve birth outcomes for people of color in Howard County to advance efforts toward reducing health disparities.

Healthy Lifestyle Workgroup
The Healthy Lifestyle Workgroup combine the efforts of the previous LHIC Food Security Committee, Chronic Disease Prevention and Management Group and Movement Group. The goal is to improve health though education and promotion of physical activity and chronic disease classes while increasing access to healthy food and healthcare.

Healthy Minds and Suicide Prevention Coalition
The Healthy Minds and Suicide Prevention Coalition works to encourage residents to have conversations about mental health and to promote help-seeking behaviors.
